Excel is a private, non-profit, CARF accredited, community based agency, dedicated to ensuring that people with disabilities receive the support needed in order to lead full lives.
Excel has a reputation for exemplary advocacy, competence and dedication, and uncompromising commitment to rights and social changes for people with disabilities. We support the disability rights movement, and we believe in education, self-advocacy, and self-directed services. Excel works closely with people and programs to ensure the needs of each individual are met, while assuring compliance with State guidelines.
Excel's case management services are limited to DD waiver program particpants.
We contract with the state of New Mexico to provide service coordination through the Developmental Disabilities Medicaid Waiver (DD Waiver).
Excel is the only full time case management agency located in the Farmington area. From this one office, we provide services to individuals receiving DD waiver services in San Juan, McKinley, and Cibola Counties. Currently, we provide case management service to more than 175 people in three counties.
