Excel Case Management, Inc.
Is a private non-profit, CARF accredited, community based agency, dedicated to
ensuring that people with disabilities receive the support needed in order to lead full
lives. We contract with the State of New Mexico to provide service coordination through
the Developmental Disabilities Medicaid Waiver (DD Waiver), Disabled & Elderly
Medicaid Waiver (D&E Waiver) and the Traumatic Brain Injury Program (TBI).
Excel has a reputation for exemplary advocacy, competence and dedication, and
uncompromising commitment to rights and social changes for people with disabilities.
We support the disability rights movement, and we believe in education, self-advocacy,
and self-directed services. Excel works closely with people and programs to ensure
that the needs of each individual are met, while assuring compliance with State
guidelines.
Excel Case Management is the only full time case management agency located in the
Farmington metro area. We are also are the only TBI case management agency in
San Juan, McKinley and Cibola Counties. Currently we provide case management
services to more then 300 people in three counties.
